Welcome to the St. John Paul II Faith Formation Program grades 1-12. To register your children for the program, please click on the link(s) below and complete the required forms. You may print & scan your forms to Maria at tejedam@sjp2cs.org or print & mail your forms to: St. John Paul II Parish, 3307 S. 10th Street, Milwaukee, WI 53215. PAYMENT MUST BE SUBMITTED BEFORE JULY 31 . An added plus, you can make your payment through WeShare, just click on the link. It's that simple! PLEASE PRINT LEGIBLY. Once your forms and payment are received you will receive an acknowledgement that your child(ren) is/are approved for the Faith Formation Program.
